Highland Home Carers is seeking a Lead Care Practitioner to deliver person-centred home care in Cromarty on the Black Isle. The role is salaried, with travel time and mileage paid; you will have access to a pool car or claim mileage if local.

You will work closely with the Rural Services Team to support individuals to live independently at home. The ideal candidate has substantial social care experience, strong assessment and care-planning skills, and can work independently as well as within a

Qualificações

• Significant experience in a social care setting.
• Strong assessment, care planning and person-centred practice skills.
• Confident working independently and collaboratively.
• Excellent communication and organisational abilities.
• Full and valid UK Driving Licence.
• Access to own vehicle preferred.
• SVQ Level 3 in Social Services & Health Care is desirable but not essential, training provided.

Responsabilidades

• Deliver responsive, high-quality care in a home-care setting.
• Conduct person-centred assessments and develop outcome-focused care plans.
• Coordinate with NHS professionals and partner agencies.
• Make timely referrals to ensure appropriate support.
• Manage your own workload and participate in on-call duties.
• Embed HHC values in everyday practice.
