Job summary

Band 4 Cardiac Rehabilitation Exercise Co-Ordinator at Berkshire Healthcare NHS Foundation Trust will assess patients and lead group cardiac rehabilitation sessions in line with BACPR guidelines, considering risks, co-morbidities and barriers. You will monitor vitals, adapt plans, and refer to the MDT when appropriate, while educating patients and carers about exercise and healthy lifestyles.

Responsibilities

• Assess risk stratification and exercise tolerance using BACPR guidance.
• Lead and deliver group cardiac rehabilitation sessions in line with BACPR guidelines.
• Monitor vital signs and modify treatment or refer to MDT as needed.
• Communicate complex information to patients, carers and staff with sensitivity and clarity.
• Educate patients, carers and significant others on exercise benefits and healthy lifestyle.
• Provide cover for other team members and supervise students/volunteers as required.
• Maintain accurate electronic patient records for the cardiac rehabilitation caseload.
