Job summary

Oxleas NHS Foundation Trust is seeking an Advanced Clinical Practitioner to lead the Primary Care team within a prison setting. The post requires a Masters-level ACP, autonomous practice, and leadership experience in prison or secure settings, with collaboration to improve outcomes and deliver best care for patients.

Qualifications

• Masters level Advanced Clinical Practice qualification (ACP).
• V300 Independent prescriber registration or equivalent.
• Autonomous practitioner with broad clinical scope including undifferentiated patients.

Responsibilities

• Lead the Primary Care team within the integrated prison healthcare service.
• Deliver high-quality primary care interventions and promote evidence-based practice.
• Provide clinical leadership and supervision to staff with annual appraisal.
• Develop and implement trauma-informed, patient-centred care planning.
• Collaborate with GPs, psychiatrists and other clinicians to review service needs.
