Essex County Council is seeking a part-time Case Manager to join the Transforming Care Programme. This role focuses on improving health outcomes for individuals with learning disabilities and autism, ensuring community support and preventing institutional care. Candidates should have relevant qualifications and experience in health and social care.
This is a part-time opportunity for 22.2 hours per week, with a pro-rata salary range of £26,058.60 to £30,657 per annum.
The Opportunity
The Transforming Care Programme is now part of a broader approach to Health Equalities for Autistic Children, Young People, Adults, and those with learning disabilities. The team works on behalf of the Collaborative of 3 ICSs and 3 Local Authorities across Southend, Essex, and Thurrock (SET) to transform the health and social care system, aiming to improve health outcomes for people with learning disabilities and/or autism (LD/A).
The Case Manager role is crucial to this transformation, bridging strategic and operational aspects of programme delivery, aligned with the NHS Long Term Plan for individuals with the greatest vulnerabilities at risk of institutionalized care.
This role operates within a hosted team, collaborating with health and social care to keep individuals safe in the community, prevent unnecessary hospital admissions, and facilitate timely discharges.
It has specific responsibilities for enabling preventative and integrated approaches for autistic adults and/or those with learning disabilities who exhibit challenging behaviors, including those with mental health conditions. The role ensures community services are coordinated to support individuals to remain in the community, avoid institutional care, and manage discharges effectively.
Key duties include providing specialist advice to health professionals and social workers, demonstrating expert practice in complex situations, supporting colleagues to mitigate risks, making independent decisions, and working across services (health, social care, education) to remove barriers and resolve issues.

This role involves travel around Essex and beyond. A driving license and access to a vehicle, or the ability to meet mobility requirements through other means, are essential.
