A part-time Case Manager role within a transformative health initiative for autistic individuals and those with learning disabilities. £43,431 to £51,095 per annum (FTE)
Location: Chelmsford
Working Style: Anywhere worker (expectation to be office based 1 day per month depending on business need)
This is a part-time opportunity for 22.2 hours. The pro-rata salary range is £26,058.60 to £30,657 per annum.
The Transforming Care Programme is now part of a broader approach to Health Equalities for Autistic Children, Young People, and Adults, including those with learning disabilities. The team works on behalf of the Collaborative of 3 ICSs and 3 Local Authorities across Southend Essex and Thurrock (SET) to transform the health and social care system, aiming to improve health outcomes for people with learning disabilities and/or autism (LD/A).
The Case Manager role is vital to this transformation, bridging strategic and operational aspects of programme delivery, aligned with the NHS Long Term Plan, especially for those with the greatest vulnerabilities at risk of institutional care.
This role operates within a hosted team, working on behalf of health and social care to keep people safe in the community, prevent unnecessary admissions to mental health beds, and facilitate timely discharges.
It carries specific responsibility for enabling preventative and integrated approaches for autistic adults and/or those with learning disabilities who display challenging behaviors, including those with mental health conditions. The role ensures community services are coordinated to support community living, avoid inpatient admissions, and promote well-managed discharges from inpatient services.
The role involves providing specialist advice to health professionals and social workers, demonstrating expert practice in complex situations, working across services (health, social care, education), making independent decisions, and troubleshooting to remove barriers and resolve issues.
