Job summary

Provide CIC in Braintree is seeking a Band 6 Community Nurse on a permanent full-time basis (37.5 hours per week). The post works with the community matron to plan, allocate and review patient care across the Braintree area, with caseload management and care planning responsibilities.

Applicants must be a registered NMC nurse with CPD, hold a nursing degree or equivalent, and ideally be a Qualified Nurse Prescriber or working towards. NHS Pension is offered; closing date 31 August 2026.

Responsibilities

  • Maintain knowledge and competences per NMC Code and PREPP.

Job Overview The post holder will work closely with the community matron to plan, allocate and review patient care. Caseload management is an integral part of this role to ensure that patients are receiving care from the most appropriate service to meet their needs. The post holder will lead on and participate in the assessment, implementation and evaluation of care and be committed to the development of the Primary Health Care Team. He/she may be designated to take charge of the team in the absence of the community matron. He/she will be expected to work across Braintree ICT and if required to support other bases within the team. Main duties of the job

  • Caseload management, including allocation and review of visit frequency etc.
  • Formulation and review of care plans.
  • Evaluation of care including first visits.
  • Ability to problem solve.
  • To promote health by educating/teaching patients and carers in line with the organizations philosophy of Health Promotion.
  • Liaise effectively with other members of the primary care team and social services and ensure an efficient system of communication with other agencies in planning and implementing community care.
  • Participate in the development of care co-ordination, and complex discharges home, including palliative care.
  • Maintain and develop knowledge, skills and competence to ensure patients’ individual needs are met in accordance with the NMC Code of Professional Conduct and PREPP.
  • Knowledge of current primary care issues and political changes.
  • Develop, maintain and promote effective communication networks for multi-disciplinary teams.
