Job summary

Cambridgeshire and Peterborough NHS Foundation Trust (CPFT) is seeking a Band 5 Community Nurse to join the Ely Community Nursing Team. The role is permanent, with both full-time and part-time hours, and a 37.5 hour week including weekends. Salary £32,073–£39,043 per annum (pro rata) with annual pay in pounds.

We are looking for a Band 5 Community Nurse to work within our Ely Community Nursing Team covering Ely and surrounding villages

In these roles, you would be working at part of the Neighbourhood team which includes District/Community Nurses, Physiotherapists, Occupational Therapists, Mental Health Nurses and Team coordinators. Linking closely with colleagues including general practitioners and other members of the multidisciplinary team to support excellent nursing care at home, reducing the necessity for acute hospital admission.

Main duties of the job

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. You will be responsible for:

• Keeping the patient at the centre of care, ensuring patient and carer participation in decision making

• Providing a high standard of nursing intervention within a patient's own home, with access to Specialist and Advanced Practitioners when required
• Working as part of a 24/7 service and multidisciplinary team

• To be professionally and legally accountable for all aspects for your own and delegated work.

• To undertake holistic assessment (including those with complex presentations and multi pathologies) making use of clinical reasoning skills.
