IUC Clinical Practitioner – BrisDoc Healthcare Services

Job summary

This exciting, variable, and autonomous role requires the post holder to work effectively within their professional boundaries, focusing on the management of routine and urgent patient needs. You will be responsible for the safe application of:

  • History taking
  • Clinical assessment (remote and face-to-face)
  • Critical thinking and decision making
  • Diagnosis
  • Treatment (including prescribing where able); non-prescribers will be supported with prescribing requirements via the Clinical Coordinator in IUC or prescribing colleagues in other clinical areas
  • Management (including safety netting)
  • Referral

The Clinical Practitioner will provide exemplary care for their patients, meeting population health needs. This includes consultations with patients from various dispositions, such as 111, primary care (1, 2, 6, 12, and 24-hour timescales), 999, Emergency Department, and Emergency Treatment Centre, as well as those presenting with routine or urgent needs. BrisDoc practice services focus on providing high-quality, accessible healthcare to vulnerable, marginalized, and excluded groups. Clinicians should aim to improve health inclusion and remove barriers to holistic healthcare.

Main duties of the job

  • Assess, diagnose, plan, implement, and evaluate treatment/interventions and care for patients with undifferentiated diagnoses, complex needs, acute and chronic conditions, including physical, mental health, and psycho-social needs
  • Clinically examine and assess patient needs from physiological and psychological perspectives, and plan care accordingly
  • Diagnose and manage patient presentations, integrating drug and non-drug treatments into management plans
  • Support patient adherence to prescribed treatments
  • Provide information on medication regimens, side-effects, and interactions
  • Prioritize health problems and intervene appropriately, including initiation of emergency care when needed
  • Support health promotion strategies and principles of self-care
  • Provide face-to-face and remote clinical assessments, treatments, and management plans
  • Offer clinical support to other healthcare professionals and operational colleagues
  • Undertake home visits as required
  • Develop and ensure robust safety netting and risk management for patients
  • Make effective referrals to system partners and other care providers for holistic patient care
