We are looking to appoint a qualified IST nurse to our Band 5-6 development post in Buckinghamshire. The post holder will be working over a 7 day rota, and this is a permanent position.
IST nursing work within the Specialist Learning Disability Service alongside the MDT including; Psychiatry, Psychologists, Physiotherapists, Occupational Therapists & Speech and Language Therapists, & Arts Therapists.
You will initially be employed as Band 5 whilst you are working through your Band 6 competencies. You will be expected to demonstrate these competently in practice. Please be aware that the expected time to complete competencies will be approximately 6-8 months. Following successful achievement of meeting the competencies, your salary will be reviewed and your pay band confirmed at band 6 pay step 1.
In the unlikely event that you didn’t meet the competencies, an action plan would be put in place to support you to address the competency gaps; you would remain as a Band 5 and the opportunity for a Band 6 at this time will not be confirmed.
We aim to provide a high-quality nursing service across the county of Buckinghamshire, providing care in line with our clinical care pathways. This post is suitable for newly qualified nurses.
Identify the health needs of people with learning disability referred to the service, using specialist clinical skills and knowledge in the following areas: epilepsy, dementia, mental health, behaviour difficulties, autism, syndrome specific health needs, medication and complex health needs associated with learning disability.
Undertake nursing assessment, specialist mental health and learning disability assessments (initially under supervision), care planning and evaluation, and risk assessment and management.
Provide a range of clinical interventions and treatments appropriate to the individual needs of service
Deliver care that is person centered and recovery focused, reflecting current best practice.
Provide education, advice and guidance to service users and their carers, and other professionals.
Responsible for the administration, carriage, storage and safe disposal of medication and equipment used in the community, in accordance with Trust
Participate in developing health promotion initiatives, working towards reducing health inequalities.
Take appropriate action in relation to safeguarding vulnerable adults, where necessary.
Participate in the Trust’s adverse incidents reporting systems and comply with the Trust’s procedure and techniques for managing
