• Triage & Assessment: timely, strengths-based assessments of need, risk and safeguarding for adults and carers.
• Screen and prioritise incoming contacts and referrals.
• Care Planning: develop and implement person-centred care and support plans to promote independence and wellbeing.
• Safeguarding: carry out safeguarding enquiries and escalate risks per statutory legislation and local procedures.
