Job summary

Central London Community Healthcare NHS Trust is seeking a Central Neighbourhood GP to join the Haringey Integrated Neighbourhood Team (INT). The post emphasises integrated care and working across agencies to improve outcomes for patients with complex needs in a preventative, person-centred model.

You will provide clinical input at weekly Complex Proactive MDTs, support MACC neighbourhood teams, and act as a primary care link clinician to strengthen patient pathways.

Qualifications

• Provide clinical input at weekly Complex Proactive MDTs (frailty and complex LTC) within the INT model.
• Provide clinical input to the MACC neighbourhood teams.
• Provide wider INT support as a primary care link clinician.
