Job summary

Dorset HealthCare University NHS Foundation Trust is seeking a Health and Social Care Co-ordinator to join the locality team in Weymouth and Portland. The role focuses on coordinating communications between primary care, community services and hospitals to deliver integrated care for frail and vulnerable individuals.

You will work with clinicians, patients, carers and statutory and voluntary agencies to develop integrated care plans, support timely discharge and prevent avoidable admissions.

Qualifications

• Excellent communication and interpersonal skills.
• Strong organisational and administrative abilities.
• Experience in health or social care environments is desirable.

Responsibilities

• Coordinate communications and meetings between primary care, community services and hospitals and other stakeholders.
• Support development of integrated and coordinated personal care plans to avoid unplanned admissions.
• Assist with discharge planning and timely patient discharge processes.
