Job summary

Mainspring Recovery is a mission-driven behavioral health organization providing Residential, PHP, and IOP care. The Intake Nurse role focuses on initial assessments, admission coordination, and ensuring medical and safety requirements are met, supporting patients and families through transitions into care.

The position requires clinical experience as an LPN or RN in Virginia, with on-site duties at Dumfries facilities and shift coverage Fri–Sun, 8:30am–7pm.

Responsibilities

• Complete medication inventories and enter info into the EMR.
• Perform nursing assessments including urine drug screens and CIWA/COWS.
• Monitor patient condition and respond to immediate healthcare needs.
• Provide emotional support and education to patients and families.
• Maintain accurate nursing documentation in the EMR.
• For Dumfries location, commute between facilities to meet PHP/LIR needs.

Compensation:

The pay range for this position is $30 - $35 per hour, set in good faith based on the responsibilities of the role, qualifications, and market conditions. Applicable shift differentials:

• $1 per hour 4:00-7:00pm
• $2 per hour 7:00pm-7:00am

Job Summary

The Intake Nurse provides knowledgeable, compassionate, and clinically sound nursing care through initial and ongoing patient assessments and care coordination for Residential, PHP/IOP, and/or LIR clients. This role is essential to ensuring a safe, thorough, and supportive admission process while maintaining accurate clinical documentation and collaboration across treatment teams.

The Intake Nurse supports patients and families during a potentially stressful transition into care and ensures all medical, safety, and compliance requirements are met upon admission.

Key Responsibilities

• Conduct initial patient assessments to obtain medical history, current health status, and presenting concerns related to admission.
• Record and document vital signs, medications, allergies, and other relevant patient information in the electronic medical record (EMR).
• Collaborate with providers and interdisciplinary team members to support individualized care planning.
• Ensure patients are comfortable and have access to necessary medications, medical supplies, and personal belongings upon admission.
• Complete medication inventories for all medications brought in by clients and accurately enter information into the EMR.
• Perform nursing assessments including, but not limited to, body searches, urine drug screens, vital signs, and withdrawal assessments (COWS/CIWA).
• Monitor patient condition and respond promptly to immediate healthcare needs during the intake and admission process.
• Provide emotional support, education, and reassurance to patients and families throughout the admission process.
• Maintain accurate, timely, and compliant nursing documentation in the EMR.
• For the Dumfries location only, commute between the main residential facility and the PHP facility (approximately 2 miles) to meet nursing needs of PHP and LIR clients, including performing daily urine drug screens.
• Maintain compliance with all nursing standards, policies, procedures, and regulatory requirements.
• Perform all other duties as assigned to support clinical operations and continuity of care.

Minimum Qualifications

Education / Credentials

• Current Virginia state Registered Nurse (RN) or Licensed Practical Nurse (LPN) license required.
• Must maintain active licensure in accordance with the standards of the Commonwealth of Virginia.

Experience

• Minimum of 6 months of Licensed Practical Nurse clinical experience.
• Intake or initial patient assessment experience required.
• CPR certification preferred; certification required upon hire if not already obtained.
• Mental health and residential substance use disorder nursing experience preferred.

Knowledge / Skills
