Job summary

Cygnet Elowen Hospital in Derbyshire is seeking a confident, compassionate Social Worker to join our inpatient team on a full-time basis. You will deliver specialist psychosocial assessments, safeguard leadership, and discharge planning support for adults with complex eating disorders and co-morbid conditions.

Qualifications

• Qualified Social Worker with current Social Work England registration.
• Experience in mental health/inpatient settings is desirable.
• Knowledge of mental health legislation and safeguarding procedures.
• Experience with discharge planning and multi-agency collaboration.
• Ability to manage complex caseloads and competing priorities.
• Excellent communication and report-writing skills.

Responsibilities

• To assist in ensuring the Company's policies and procedures are implemented and carried out within the Social Work department.
• To hold and be responsible for managing a ward caseload, ensuring individual Service Users receive appropriate therapeutic care and support in accordance with their social care needs, including those service users who may have committed serious offences such as homicide, arson, violent or sexual assaults against adults and children
• To assist with multi-disciplinary assessments of service users through referral to discharge
• To refer and support Service Users with assessment under the Care Act 2014 and for support under Section 117 of the Mental Health Act 1983
• To support Family/Carers using appropriate assessments and to deliver advise, guidance and information; ensuring the Triangle of Care principles are used as appropriate at your service
• To attend all statutory and mandatory training as required
• To provide an evidence-based, recovery-focused, Person-Centred Social Work service.
• To assess and collaborate with Service Users in planning and implementing care plans and other assessment and support tools
• To assist service users through a range of Social work tasks and responsibilities. This may include individual home risk assessments, group work, and liaison with community professionals and the Ministry of Justice around discharge and safety planning.
• To liaise effectively with other professionals and significant others involved in a Service User's care, ensuring integrated service delivery and to assist each Service User to attain his or her maximum level of functioning and independence.
• To keep up to date with developments in social work related to the client group
