Barking Havering and Redbridge University Hospitals NHS Trust is recruiting a Band 5 Medicines Management Pharmacy Technician (MMPT) to support inpatient medicines optimisation across St Ann’s Hospital and Chase Farm Hospital. The role is patient-facing, with opportunities to develop medicines reconciliation, prescribing support and discharge planning.

Qualifications

• Registered pharmacy technician.
• Band 5 Medicines Management Pharmacy Technician (MMPT).
• Experience in medicines reconciliation and patient-facing roles.

Responsibilities

• Work directly with patients to understand medicines and identify issues.
• Undertake medicines reconciliation and detailed medication histories.
• Assess patients’ own drugs (PODs) and help ensure medicines are available when needed.
• Provide high-quality medicines counselling and support patients to understand their treatment.
• Play an active role in discharge planning, including DMS referrals.
• Collaborate with pharmacists and multidisciplinary teams to resolve medicines-related problems.
• Provide ward-based medicines management and promote safe use of medicines.
• Use accredited checking skills to support safe and efficient services.
• Provide cross-site support and develop trainees and colleagues.
