Integrated Care 24 (IC24) is seeking an Advanced Nurse Practitioner for Hastings Primary Care Hub. You will assess, diagnose and treat patients at first contact, autonomously, with prescribing responsibilities and a focus on reducing health inequalities.

You will lead clinical governance within a multidisciplinary team, navigate patients to the right care, and collaborate with local partners to support vulnerable populations including those experiencing homelessness.

Qualifications

• Registered nurse with current NMC registration.
• Independent Prescriber qualification.
• Physical Assessment of Adults module and Minor Illness qualification.
• At least two years' experience in General Practice or Urgent Care.
• Excellent clinical assessment and decision‑making skills.
• Confident working autonomously and managing clinical risk.
• Experience of service improvement, audits and clinical governance.
• Effective communication with patients, colleagues and partners.
• Commitment to continual professional development and evidence‑based practice.
• Thrives in a collaborative, cross‑service environment.

Responsabilités

• Deliver advanced clinical assessment, diagnosis and treatment at the point of first contact.
• Prescribe medications as an Independent Prescriber following assessment and best practice.
• Work with unregistered patients and those experiencing health inequalities.
• Navigate patients to the most appropriate care pathway.
• Contribute to audits, quality improvement and service development.
• Collaborate with internal teams and external partners to improve patient care.
• Help develop the Hastings Primary Care Hub and share best practice.
