Job summary

A2Dominion is seeking a dedicated support worker to deliver high quality person-centred assistance to customers in supported accommodation. You will help tenants sustain tenancies by promoting independence, freedom and choice.

Responsibilities include assessing referrals, liaising with health professionals and agencies, explaining tenancy rights, and delivering planned support. A driving licence and own transport are desirable to cover schemes in Newbury and Thatcham.

Qualifications

• Educated to GCSE level or equivalent in Maths and English.
• Ability to work under own initiative and build relationships.
• Experience supporting diverse client groups; experience with substance misuse and mental health is valued.

Responsibilities

• Assess referrals and engage with prospective clients using a person-centered approach.
• Liaise with health professionals, agencies and colleagues to ensure cooperation.
• Explain financial obligations, rights and responsibilities under tenancy/occupation arrangements and related procedures.
• Agree and review Independent Living Plans and Risk Assessments; deliver contracted hours of support.
• Report safeguarding concerns using correct procedures and attend meetings as required.
• Build strong relationships with partner agencies and stakeholders.
