LiveWest is seeking a dedicated Support Worker to empower individuals with complex needs to live independently and safely within their communities. You will provide person-centred assistance, build relationships with customers and partner agencies, and promote participation in education, social activities and employment.

The role is based at our New Start service in Truro, 37 hours per week on a rolling 4 week rota, Monday to Sunday 8am–9pm, including bank holidays and weekends.

• Experience of risk assessing and risk management.
• Great communicator with the ability to build rapport.
• Accurate record keeping and computer skills.
• Confident in lone working and a good team player.

Responsabilidades

• Deliver Tailored Support: provide responsive, person-centred assistance to individuals with complex needs, helping them achieve positive outcomes and independence.
• Foster Collaboration: build open and trusting relationships with customers, colleagues and partner agencies to ensure seamless and comprehensive support.
• Promote Participation: encourage individuals to engage in social, educational and employment opportunities, preventing isolation and promoting self-determination.
• Manage Risks Effectively: conduct thorough risk assessments and maintain professional boundaries, ensuring health, safety and safeguarding policies.
