Job summary

Kent Community Health NHS Foundation Trust is seeking a Healthcare Assistant to join our Urgent Care Services team, delivering high-quality care in patients' homes alongside nurses, therapists and other professionals.

You will support delegated care plans, perform trained clinical interventions, monitor progress, record vital signs, glucose, urinalysis and wound care, and help individuals regain independence.

Responsibilities

• Support delegated care plans, undertaking clinical interventions you've been trained and assessed as competent to perform.
• Monitor patient progress and record vital signs, glucose monitoring, routine urinalysis, wound care, falls prevention support, equipment provision and helping individuals regain independence with everyday activities.
• Contribute to therapy programmes, support individual and group sessions, and provide feedback to clinicians to inform ongoing care planning.
• Follow established procedures and best practice to ensure safe, effective care while maintaining dignity, choice and independence.
