Job summary

Suffolk County Council invites applications for an Independence and Wellbeing Practitioner to support the Home First team in assessing customers and delivering person-centred outcomes. You will identify high-quality goals, build strong relationships with customers and work with social care, health partners and the voluntary sector to enable integrated care.

Your role and responsibilities

  • Support the Home First team with customer assessments and reviews.

You will need

  • strong communication skills when working with customers, their families or advocates, and professional partners
  • the ability and energy to respond to urgent work as part of a team system
  • resilience under pressure and the ability to remain positive when facing challenges.

The team

Home First provides a short-term reablement care and support service to people living in their own homes.
