Job summary

Salford Council is seeking a Best Start Inclusion Practitioner to join the Starting Life Well Service and support inclusive family hub services for babies, children, young people and families with additional needs across Salford.

The role focuses on early identification, inclusive practice and targeted support within universal settings, working with families and practitioners to promote participation, confidence and positive developmental outcomes for children and families.

Responsibilities

• Work with babies, children, young people and families, including children aged 019 and up to 25 with SEND.
• Support inclusive universal activity across Family Hubs, early years settings and community venues.
• Provide information, advice and guidance to families and early years practitioners.
• Help identify emerging needs early, using agreed approaches and tools.
• Contribute to appropriate onward referrals and pathways to additional support.
• Promote smooth, inclusive and well-coordinated transitions into and between early years provision, including PVI settings.
• Work closely with Family Hubs Workers, early years providers and wider partners to strengthen inclusive practice.
• Maintain accurate records and use family voice and feedback to support continuous improvement.
