Job summary

Advantmed is seeking a licensed Nurse Practitioner to perform structured in-home wellness and risk-adjustment assessments as a 1099 PRN contractor. The role offers flexible weekdays with no evenings or weekends, and visits are documented in the EMR with strong operational support.

Ideal candidates have 3+ years of patient care, experience with EMR, and are willing to obtain additional licensure if needed. Travel within a 55-mile radius is required; compensation per completed visit includes

Qualifications

• Active NP license in the state you will work in.
• 3+ years of patient care experience preferred.
• Comfortable with EMR systems and independent work.
• Willing to obtain additional licensure if needed (supported by Advantmed).

Responsibilities

• Conduct in-home Annual Wellness Visits and chronic condition assessments.
• Perform focused histories, exams, vitals, medication reviews, and screenings.
• Document visits in the EMR to support value-based care and risk adjustment.
• Travel within a 55-mile radius of the assigned county.
