Job summary

South London and Maudsley NHS Foundation Trust operates a Croydon Home Treatment Team delivering 24/7 crisis assessment and intensive home treatment to prevent hospital admission where possible and to facilitate earlier discharge.

The post holder will work as a Registered Mental Health Nurse, Social Worker or Occupational Therapist providing care from history taking, through assessment and treatment, to evaluation for service users in crisis, and will also support carers within the home

Qualifications

• A Registered Mental Health Nurse, Social Worker or Occupational Therapist with clinical background is required.

Responsibilities

• Provide community/home environment-based support to service users in crisis to promote recovery.
• Support carers of service users receiving home treatment and work within a multi-professional team.
