• Conducting comprehensive assessments to identify the needs of children with disabilities and their families.
• Developing and implementing tailored support plans in collaboration with families and other professionals.
• Providing advocacy and support to families to navigate services and access resources.
• Monitoring and reviewing progress to ensure the effectiveness of interventions and making adjustments as necessary.
• Building strong relationships with children, families, and partner agencies to foster a supportive network.
