Job summary
Herefordshire Council, Herefordshire Community Wellbeing is seeking a dynamic Social Worker to deliver adult social care with a strengths-based approach. You will undertake Care Act Assessments, person-centred support planning, and Mental Capacity Assessments while engaging with diverse adult populations in a fast-paced, supportive environment.

Qualifications
- Registered Social Worker with Social Work England.
- Proven ability to assess and plan care for adults.
- Understanding of current legislation and its impact.
Responsibilities
- Perform Care Act Assessments and person-centred support planning.
- Conduct Mental Capacity Assessments and Section 42 Enquiries.
- Work with diverse adult populations including physical/learning disabilities and mental health needs.
- Collaborate with partner agencies to ensure holistic care.
- Participate in CHC and s117 work and engage in supervision and professional development.
