Job summary

West Sussex County Council seeks an assistant care manager to support the Lifelong Services team in Horsham. The role involves providing social work support, including assessment, planning and review, under the Supporting Lives, Connecting People framework.

You will manage a challenging portfolio of casework and collaborate with health, community and third sector colleagues. The post is 30 hours per week, permanent, with hybrid working from the Horsham office base.

Qualifications

  • GCSE English and Maths or equivalent literacy/numeracy.
  • NVQ Level 3 in Care (Desirable).
  • Experience in Social Care environment.
  • Experience in multi-agency settings.
  • Knowledge of policies and procedures relevant to Social Work.

Responsibilities

  • Provide social work support including assessment, planning and review under the Supporting Lives, Connecting People framework.
  • Manage a portfolio of casework and attend multi-disciplinary meetings.
  • Work in partnership with Health, Community and Third Sector colleagues.
  • Adapt to various locations across Horsham and surrounding areas.

About The Job

Salary: £24,741 - £26,853 per annum
Working Pattern: Part time, 30 hours per week
Contract Length: Permanent
Location: Horsham office base with hybrid working

In this exciting opportunity, you will, under guidance and regular supervision provide a social work support service to those people who access or need the service, to include assessment, planning and review within the Supporting Lives, Connecting People framework. You will manage a challenging yet rewarding portfolio of casework, attending multi-disciplinary meetings and working in partnership with our Health, Community and Third Sector colleagues.

We are looking for a self-motivated, collaborative new assistant care manager to join our team. This is an excellent opportunity to make a significant difference to our service users lives, although challenging the rewards by far outweigh those challenges as you will always strive for a positive outcome. No two days will look the same; you'll be working in at various locations across Horsham and the surrounding areas with numerous colleagues and customers from all walks of life. You will need to take an organised approach whilst exercising your expertise with knowledge of policies and legislations to support those in need.

Experience and Skills

Key Skills (please cover within your application):

  • Ability to analyse and interpret information and prioritise needs in order to make recommendations/decisions which will have a positive outcome.
  • Ability to work independently and as part of a team, working under pressure and to tight deadlines.
  • Ability to work in a multi-agency environment to deliver a service to those people who access or need the service.
  • Ability to communicate complicated or sensitive information to a range of audiences to achieve positive outcomes; to be able to communicate clearly with customers reflecting the service objectives and standards.

Qualifications and/or experience:

  • GCSE English and Maths or equivalent qualification or experience operating at this level of literacy and numeracy.
  • NVQ Level 3 in Care. Desirable
  • Experience of providing support/working with people in a Social Care environment.
  • Experience of forming effective working relationships within a multi-agency setting.
  • An awareness of the policies, practices and procedures relevant to Social Work. Experience of the assessment and review process of care packages.
  • Experience of applying relevant legislation, policies and procedures to individual pieces of work.
