The Royal Borough of Windsor and Maidenhead is recruiting a Social Care Practitioner to join the Locality Access Team (LAP). This full-time, permanent role offers hybrid working and a salary of £26,895 - £30,132 per annum.

You will complete Care Act Assessments, hold a caseload, and provide advice and signposting to residents. The LAP team supports residents with multidisciplinary care to look at needs holistically, helping prevent longer-term challenges and hospital admissions.

Qualifications

• Experience delivering Care Act Assessments.
• Experience managing a caseload in a care setting.
