Qualifications

• Experience in social care or public sector environments.

Responsibilities

• Source care from contracted providers to support physical and personal day-to-day needs, psychological support, and end of life care.
• Coordinate with Commissioning and Procurement Team to ensure service delivery meets requirements.
• Maintain clear verbal and written communication with stakeholders and service users.
