Nottingham City Council is seeking a Prevention Officer within the Nottingham Health and Care Point (NHCP) to support people in maintaining independence, wellbeing, and quality of life through targeted interventions and guidance.

You will assess needs, identify risks, and coordinate with partners to connect individuals with services, resources, and preventive solutions, promoting person-centred care and timely support.

Qualificações

• Experience of working in a health, social care, housing, community or related setting.
• The ability to gather, analyse and interpret information to assess needs and determine appropriate outcomes.
• Excellent communication and interpersonal skills and an ability to build effective working relationships at all levels.
• Experience of working collaboratively with colleagues and partner agencies.
• An understanding of safeguarding principles and the ability to recognise and respond to concerns appropriately.
• A commitment to equality, diversity and inclusion, ensuring services are accessible and responsive to individual needs.

Responsabilidades

• Assess needs, identify risks and work with internal and external partners to connect people with appropriate services and resources.
• Provide face-to-face interventions, timely information, advice and guidance to maintain independence and wellbeing.
• Promote independence and prevent escalation to more intensive services.
