Sanctuary Personnel is seeking an Occupational Therapist to join the Social Service Reablement Team in Blackpool. The role offers hybrid working, a 37-hour week, and a competitive wage of £31.89 per hour.

Responsibilities

• Conduct comprehensive occupational therapy assessments for adults with diverse needs, focusing on enhancing their independence and wellbeing.
• Craft and manage person-centred care plans, working with individuals, families, and other professionals.
• Assess and enhance adults’ functional abilities in activities of daily living, environment accessibility, and community involvement.
• Facilitate and evaluate the use of specialist equipment and reablement strategies, adhering to legislation and eligibility criteria.
• Balance independence and safety through professional risk assessments and decision-making.
• Support informed decision-making by providing specialist advice to service users, carers, and professionals.
• Facilitate smooth hospital discharges and prevent admissions by managing timely interventions and community support.
• Mentor and supervise support staff, ensuring effective service delivery and safety.
• Lead service development initiatives to foster innovation and best practice.
• Stay current with occupational therapy practices and maintain professional development requirements.
