Job summary

Leaders in Care is recruiting an Adult Locality Social Worker to join a busy community-based service in the Bangor area. The role offers a flexible hybrid pattern with 1-2 office days per week and a strong hourly rate to reflect expertise.

You will support adults to live independently at home, handling care assessments, safeguarding, and care planning while working closely with families and health professionals.

Qualifications

• A recognised Social Work qualification is required.
• Active Social Work England registration is required.
• Experience in Adult Social Care, preferably within locality/community teams.
• Ability to manage a busy caseload and undertake community visits.
• Strong assessment, safeguarding and care-planning skills.

Responsibilities

• Complete Care Act assessments, reviews and support plans.
• Manage complex caseloads within the community.
• Undertake safeguarding enquiries and risk assessments.
• Collaborate with families, health professionals, providers and partner agencies.
• Support adults to access appropriate care and resources.
• Promote independence, choice and positive outcomes.
