Job summary

Devon Partnership NHS Trust is seeking an Assistant Practitioner to join the Eating Disorder Outreach Team on a fixed-term basis until 31/03/2027. The role involves home visits and a hybrid model of face-to-face and virtual clinical contacts to support individuals with severe eating disorders, alongside their families and carers.

You will join a small MDT and contribute to psychoeducation, meal preparation and distress management, with supervision by the team manager.

Qualifications

• Foundation Degree in Health and Social Care or diploma level or equivalent is required.
• Experience of working with individuals with Mental Health presentations including Eating Disorders is desirable.
• Excellent communication and ability to work creatively with individuals, families and carers.

Responsibilities

• Work as part of a small MDT to provide treatment and interventions in adults' homes and virtually.
• Provide practical support such as meal preparation and dining alongside psychological exploration after eating and distress management.
• Support family and carers with psychoeducation and upskilling around mealtimes and distress management.
• Liaise with professionals across the Eating Disorder Pathway, community and inpatient services.
• Develop and review individual care plans and risk assessments.
