Job summary

Devon Partnership NHS Trust invites applications for an Assistant Practitioner to join the new Eating Disorder Outreach Team on a fixed-term basis until 31/03/2027. The role involves delivering care in patients’ homes and through virtual contacts, within a small MDT, to support recovery from eating disorders.

Responsibilities

• Provide treatment and interventions to adults with eating disorders in homes and virtually as part of a small MDT team.
• Deliver practical support such as meal preparation and dining while exploring feelings after eating and distress management.
• Support family and carers with psychoeducation and upskilling for mealtimes and distress management.
• Collaborate with professionals across the Eating Disorder Pathway, community and inpatient settings.
• Develop and review individual care plans and conduct risk assessments.

This new service aims toincrease support to individuals by wrapping around the care provision providedby their Community Eating Disorder Team in a supported way to promotingrecovery from an Eating Disorder.

Your clinical skills will involve working individuals with a severe and complex eating disorder providing interventions such as psycho education, meal preparation and meal support which will be overseen by the team manager. You will need to have excellent communication skills and be able to think creatively in not only supporting the individual but also their family and carers. The role will require commuting to individuals home's and will be a hybrid model of a mixture of face to face clinical contact's alongside some virtual contacts.
