Job summary

Keston and Moorings Medical Practice in Purley, Surrey, is seeking a Pharmacist with prescribing qualifications to join our hub model across established sites. You will work closely with nurses and GPs, taking responsibility for chronic disease management and clinical medication reviews, including complex polypharmacy cases.

Qualifications

• Supervise junior pharmacists and/or technicians when needed.
• Experience in chronic care conditions (asthma, COPD, hypertension, type 2 diabetes).
• Previous primary care experience delivering patient-centred care.
• Desirable: minor illness management and ACP qualification.

Responsibilities

• Conduct chronic care reviews and assist the practice with QOF.
• Carrying out virtual medication reviews for repeat prescriptions.
• Review medication queries from GPs, nurses and other healthcare professionals.
• Liaise with secondary care teams for medication-related issues as needed.
• Review hospital discharge letters and carry out medication reconciliation.
• Prescribe repeat medications and manage pathology results.

Job summaryThe successful candidate will work in our pharmacy hub across all of our established sites, offering exposure to enhanced services and modern models of care delivery. We are looking for a proactive and enthusiastic Pharmacist with prescribing qualifications, preferably with Chronic care or minor illness experience, to join our Practice to work closely with our nurses and GPs. The appointed Pharmacist would be part of a wider pharmacy team working with our associate practices within a Hub model. Main duties of the job As the role will develop the successful candidate will take responsibility for areas of Chronic Disease Management and undertake Clinical Medication Reviews, proactively manage patients with complex poly-pharmacy needs. The role would also entail providing primary support to GP staff with regards to prescriptions (this includes repeat prescribing), medication queries, complex clinical queries - please see full job description. There will also be training to manage low-level investigation results following strict protocol and supervision. We will look to provide support to develop and train the candidate to further their skills in their clinical interests. The Pharmacist would have an allocated Lead Pharmacist and GP Partner to mentor, support and guide them.
