Job summary

Cheshire and Wirral Partnership NHS Foundation Trust is recruiting a Housing Officer for the Mental Health Intensive Support Team. You will support people with complex housing needs to access and sustain suitable accommodation, working with patients, carers and housing providers to prevent homelessness and enable discharge.

You will manage cases, liaise with authorities and welfare agencies, and help individuals apply for welfare benefits while maintaining independence in the community.

Qualifications

• Good literacy and numeric skills.
• Knowledge of welfare benefits legislation and eligibility.
• Experience in tenancy sustainment and supported housing pathways.

Responsibilities

• Manage a caseload of individuals with housing needs and provide advice, support and advocacy.
• Liaise with local authorities, housing providers and welfare agencies to secure housing options.
• Support housing applications, tenancy sustainment and welfare benefits issues.
• Work within a multidisciplinary team to integrate housing considerations into care and discharge planning.
• Develop relationships with partner agencies and maintain confidentiality and accurate records.
