The London Borough of Hounslow is seeking a Housing and Health Link Worker to deliver a frontline, integrated service across housing, health and social care, focusing on vulnerable residents.

You will work with NHS partners, support hospital discharge, and assess needs with a holistic approach, maintaining on-site presence in hospital settings and helping reduce temporary accommodation use.

Formación

• Experience with Housing Act 1996 and Homelessness Reduction Act 2017.
• Experience leading multi-disciplinary housing teams.
• Ability to engage with vulnerable people and build trust.
• Knowledge of homelessness and related legislation.

Responsabilidades

• Deliver a proactive front-line service across housing, health and social care.
• Promote access to preventative services and timely access.
• Lead casework-based assessments and bespoke interventions.
• Collaborate with NHS partners to support hospital discharge.
• Maintain on-site hospital presence to prevent homelessness.
• Work to reduce use of temporary accommodation.
