Job summary

Join a supportive, experienced team; 37 hours weekly; weekend rota; 15% Attraction & Retention Payment; you'll use Care Act assessments, Mental Capacity Act decisions, and collaborate with health colleagues to achieve safe outcomes.

Qualifications

• Registered with Social Work England.
• Minimum two years' post-qualification experience.
• Confident with complexity, risk and uncertainty.
• Passionate about strengths-based practice.
• Strong communicator with people, families and professionals.
• Enthusiastic about learning and developing skills.
• Desire to make a difference.

Responsibilities

• Completing Care Act assessments and support planning.
• Working with ward teams to support safe and timely discharges.
• Identifying strengths and promoting independence for service users.
• Leading or contributing to safeguarding enquiries.
• Supporting Mental Capacity Act and Best Interest decision-making.
• Collaborating with families, carers and partner agencies.
• Managing a varied and complex caseload.
• Mentoring colleagues and developing students.
• Deputising for the Lead Practitioner when required.
