Job summary

Pharmacyupdate is recruiting an experienced Adult Social Care Team Manager for our Hospital Discharge Team based at Charing Cross Hospital. You will lead a multidisciplinary team, oversee complex cases, safeguarding and risk management, and drive high-quality, person-centred discharge planning from hospital to community support.

Job description

We are seeking an experienced Adult Social Care Team Manager to manage and develop our Hospital Discharge Team based at Charing Cross Hospital. This is a key management role overseeing a multidisciplinary team and ensuring the delivery of high-quality, person-centred services that promote well-being, choice and positive outcomes for residents.

The successful candidate will provide operational management, supervision and guidance to Social Workers and other professionals, overseeing complex casework, safeguarding concerns and risk management. You will work closely with health partners, community services and key stakeholders to support effective discharge planning and seamless transitions from hospital to community-based support.

The role requires strong knowledge of Adult Social Care legislation, including the Care Act 2014, Mental Capacity Act and safeguarding responsibilities, alongside experience of performance management, service improvement and partnership working within a Local Authority setting.
