Job summary

Westminster City Council is seeking a Social Worker to join the Hospital Discharge Team based at St Mary’s Hospital, and operating across the Boroughs of Hammersmith & Fulham, Kensington & Chelsea and Westminster. This 36-hour-per-week temporary role includes delivering timely assessments to support hospital discharge and safeguarding duties.

You will maintain professional registrations, complete CPD, and collaborate with health colleagues to maximise patient independence while ensuring

Qualifications

• CQSW/Diploma in Social Work or Degree in Social Work and registered with Social Work England.
• Knowledge of legislation, person-centred care and independence.
• Experience with older or physically disabled people in social services.
• Experience in a multi-disciplinary environment and ability to work independently.

Responsibilities

• Undertake assessments of need and develop support plans for hospital discharge.
• Carry out safeguarding investigations and MCA assessments.
• Work with carers and other professionals to manage risk and promote chosen lifestyles.
• Contribute to CPD, appraisals, audits and reflective practice in a team setting.
