Job summary

Liquid Personnel Limited. is seeking a Social Worker for its Hospital Discharge Team in Manchester. You will work with patients preparing for discharge, coordinating multi-agency plans and ensuring safeguarding and person-centred care throughout the process.

The role involves completing Care Act assessments, MCA considerations and collaboration with health, social care, and housing partners to support safe and timely discharges.

Qualifications

• Must be Social Work England registered and eligible to work in the UK.
• Hold a full UK driving licence.
• Post-qualification experience in social work desirable.
• Degree level or equivalent in Social Work required.

Responsibilities

• Provide timely, person-centred social work assessments to patients preparing for discharge.
• Assess care and support needs and identify discharge pathways and supports.
• Collaborate with patients, families, carers and multidisciplinary teams for safe discharges.
• Coordinate discharge planning with health, social care, housing and community services.
• Complete Care Act assessments and develop care plans where needed.
• Undertake Mental Capacity Act assessments and contribute to best-interest decisions.
• Identify safeguarding concerns and take appropriate actions to protect adults at risk.
• Advise patients and families to understand options and make informed care decisions.
• Arrange and coordinate packages of care, reablement or supported accommodation.
• Maintain comprehensive case records and attend discharge planning meetings.
