Responsibilities:
  1. Ensure safe and smooth discharge of patients from the hospital.
  2. Collaborate with patients, families, and healthcare professionals to coordinate post-discharge services.
  3. Develop and implement care plans tailored to individual patient needs.
  4. Act as a liaison between hospital staff and external care providers to ensure continuity of care.
  5. Address challenges to facilitate seamless patient transitions.
