* Manage complex long-term cases including safeguarding risk assessment capacity assessments MCA and Care Act compliance with exceptional attention to detail.

* Collaborate within multidisciplinary teams across health and social care driving innovation and excellence in service delivery.

Must have CHC or Hospital or Long Term Case experience. Must be able to work 2 days from office within city boundaries.
