Responsibilities of a Hospice RN / RN Case Manager:
  1. Report directly to the Administrator, Executive Director, or Patient Care Manager and maintain primary control and professional management of each patient.
  2. Act as the primary liaison between the physician, patient/family, and hospice team.
  3. Provide direct patient care services to hospice patients and supportive care to the patient’s family, managing a reasonable caseload while building trusting and compassionate relationships.
  4. Empower patient’s caregivers by educating them on care for their loved one.
  5. Manage and collaborate with an interdisciplinary team including LPNs, CNAs, and support staff to provide comfort, peace, and dignity.
  6. Evaluate patient and family needs and assist in the development and execution of the care plan.
  7. Maintain all assigned patient clinical records.
  8. Participate in company quality assessment and performance improvement programs.
About You

Requirements for Hospice Registered Nurse / RN Case Manager:

  • Compassionate, kind, and empathetic towards patients and families.
  • Comfortable providing care in the patient’s home.
  • Flexible and able to support others in a crisis.
  • RN license in the requested state or eligibility to obtain one.
  • At least 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
  • Additional experience in hospital staff RN, Med Surg, PACU, ER, critical care, or related fields is a plus.
  • Valid driver’s license and auto insurance.
  • Current CPR certification.
