Job summary

Danone UK & Ireland is recruiting a Nutricia Homeward Nurse to deliver in-home care and training for patients tube-fed at home. You will partner with carers and healthcare professionals across Berkshire and Frimley, building trusted relationships and supporting safe discharges from hospital.

You will work independently, manage your diary, and travel to patient homes with a company car and fuel card. A Registered Nurse with NHS knowledge and 12 months' experience is required, plus a full UK

Qualifications

• Ability to work independently with strong diary management and scheduling.
• Excellent communication with patients, carers and healthcare professionals.
• Understanding of the UK healthcare market and NHS pathways.
• Registered Nurse with valid UK registration and experience.

Responsibilities

• Deliver in-home training to patients, carers and HCPs for safe discharge and ongoing care at home.
• Perform regular patient reviews and be available to answer questions and troubleshoot issues.
• Assist in preventing unnecessary hospital stays and support tube changes within the community.
