Inizio Engage in the UK is seeking an experienced Community Nurse to join our Clinical Homecare Nursing Team. You will deliver patient-centred parenteral nutrition and IV therapy in patients' homes, working closely with NHS colleagues to ensure safe transitions from hospital to home.

The role requires a current NMC registration, a drive with UK licence, and at least one year post-registration experience in community or homecare settings.

Qualifications

• Current NMC Registration.
• Degree-level qualification or equivalent experience.
• Minimum one year post-registration nursing experience.
• Evidence of continuing professional development.
• Current valid UK driving licence.
• Experience in CVAD and/or Parenteral Nutrition services.
• Pharmaceutical or healthcare industry experience.
• Experience delivering patient education and support programmes.
• Experience working within community, homecare, acute or NHS healthcare settings.

Responsabilités

• Deliver safe, patient-centred care in patients' homes and community settings.
• Administer Parenteral Nutrition, IV therapies and clinical interventions.
• Assess patients and escalate clinical concerns appropriately.
• Support treatment initiation and transitions from hospital to home.
• Educate and empower patients, families and carers to manage treatment.
• Provide clinical advice and build relationships with patients and healthcare professionals.
• Collaborate with NHS colleagues to ensure continuity of care.
• Maintain accurate clinical records and adhere to governance and policies.
• Support safety, quality improvement and KPI achievement.
• Maintain clinical competence through training and CPD.
