Job summary

Lincolnshire Community Health Service NHS Trust is recruiting for a Promoting Independence Therapy Assistant- Sleaford (Band 3). The post involves providing rehabilitation and reablement to patients in their own homes following hospital discharge, promoting independence and assessing longer-term needs.

Based on the Discharge to Assess principles, roles cover Lincolnshire with local teams. The service runs 08:00–20:00, seven days a week; part-time hours can be considered, and an apprenticeship

Qualifications

• 5 GCSEs at grade 4 or above, or equivalent
• A L3 Vocational Healthcare Qualification or Equivalent
• Moving and handling training/experience

Responsibilities

• Provide rehabilitation and reablement to patients at home after discharge.
• Promote independence and contribute to assessments of future needs.
• Support a multi-disciplinary team to prevent dependence.
• Undertake environmental assessment within the patient’s home.
• Communicate effectively with patients, carers and health professionals.
• Work with other agencies to determine long-term care needs.
