Job summary

Locala Health And Wellbeing is seeking an experienced Assistant Practitioner to join the Home Enteral Nutrition (HEN) Team in South Kirklees on a permanent basis. You will support patients with complex nutritional needs, collaborate with Dietitians and other community professionals, and help deliver safe, person-centred care in patients' homes.

Qualifications

• Experience in an Assistant Practitioner role with nutrition/dietetic understanding.
• Excellent communication and interpersonal skills with patients, families and colleagues.
• Strong organisational abilities to handle clinical data and admin tasks.
• Compassionate, patient-centred approach when supporting people with complex health needs.

Responsibilities

• Provide clinical and administrative support to the HEN Team delivering high-quality service.
• Support nutritional care by collecting patient information to inform feed regimes.
• Co-ordinate nutritional supplies and prescriptions per Dietitians' guidance.
• Register patients on systems and maintain accurate clinical records in SystmOne/Nutricia Homeward.
• Monitor patient progress with scales and ensure records are up to date.
• Communicate adaptively to meet individual needs, including AAC considerations.
• Assist with discharge planning and transitions home with appropriate equipment.
• Attend MDTs and share relevant information to support coordinated care.
