NHS Scotland is inviting applications for a Reablement Worker in the Nairn & Ardersier Reablement Care @ Home Team. The role focuses on home assessments, enablement planning, and supporting individuals to remain independent at home, aiding hospital discharge and anticipatory care planning.

Applicants must be registered with the SSSC within 3 months of appointment and have a driving licence with access to a car. Gaelic speakers welcome; closing dates apply.

Formación

• Registered with the Scottish Social Service Council within 3 months of appointment.
• Able to travel to patients' homes and work in the community.

Responsabilidades

• Assess and plan enablement approaches for individuals and families.
• Identify outcome plans to maximise independence and home-based care.
• Support prompt discharge from hospital and anticipatory care planning to prevent readmission.
