This permanent role is 30 hours per week, with a pro-rata salary of £29,542–£32,046 per annum and allowances for evening and weekend work. A driving licence is desirable and a DBS check is required.

About the Role

In this rewarding role, you will support vulnerable people to maintain and regain their independence, enabling them to live safely and comfortably in their own homes. Using a strengths-based approach, you will help individuals improve their quality of life and achieve their personal goals.

You will also play a key part in supporting people as they transition home from hospital, working within Discharge to Assess pathways, as well as providing complex and palliative care support where needed.

Previous experience in care is beneficial but not essential—we welcome applicants with the right values and a willingness to learn.

Working Hours

This role involves working on a rota basis, including weekends, bank holidays, and shift patterns.
