Job summary

NHS National Services Scotland is seeking an assistant practitioner to support the Community Rehabilitation Team in Levenmouth. You will work with Occupational Therapy and Physiotherapy staff to deliver therapeutic programmes in patients' own homes, helping prevent hospital admissions and enabling early discharge.

The role requires an HNC/HND in Health and Social Care or SCQF level 8 and prior experience as a therapy support worker, preferably with older adults.

Qualifications

• Essential experience as a therapy support worker.
• Experience working with older people is essential.

Responsibilities

• Deliver therapeutic treatment programmes in patients' own homes as part of the Community Rehabilitation Team.
• Hold own caseload, assess, progress and discharge autonomously within parameters.
